When filling out the Consultation Form, please be as thorough as possible and provide us with adequate contact and personal information to make the treatment and consultation process smoother. The Form is lengthy, exhaustive and deliberately designed to help the patient channelize his/her thoughts in a manner that can assist us in arriving at a definite conclusion about your health issues.

Please be patient while filling the Form. If you are not comfortable or clear about some question, skip it. It can always be discussed with your physician during case-taking. A doctor from the Clinic will contact you during your preferred contact time and discuss your case with you either on tele or a video-chat through Skype/Facetime.
